After previously beating this game on the "sane" (normal) difficulty, I decided to try out "insane". It was anything but that.
The good news is that I discovered some interesting things that either were only on the insane mode or that I completely missed the first time through.
Also, the mini-map/radar is gone. Surprisingly, this is a good thing in some ways. You don't get to see the enemies, but since the mini-map has no concept of height, I found it annoying hiding out when I thought an enemy was right on top of me only to figure out that he was a floor away and could not see me. So in that sense, good riddance to the minimap. On the other hand, I liked the markers for the environment kills, since those were the most fun. Too bad about that.
The bad news is that the game is not any harder and if you played "sane" first, it is actually pretty easy. I only noticed one minor change in a level toward the end, but otherwise it's all pretty much the same stuff. I haven't finished the bonus level yet, but it will likely be the same too.
All in all, after playing through the first time, the amount of new and interesting things did not really justify the second pass, so the game loses a point there.
Bottom line: Manhunt 2 is a one-trick pony and a rental at best.
